Don't use benzodiazepines or other sedative-hypnotics in older adults as the first choice for insomnia, agitation, or delirium. Setting: older adult (typically 65+) with insomnia, agitation, or delirium Why: Older adults are more sensitive to benzodiazepines, with markedly increased risk of cognitive impairment, falls, fractures, and motor-vehicle crashes. Consider instead: non-pharmacological measures first; treat underlying causes Source: American Geriatrics Society (Choosing Wisely, US, 2013).
