Don't use systemic (oral) antibiotics for mild comedonal or mildly inflammatory acne; topical therapy is first-line. Setting: mild acne — comedonal or with few inflammatory papules/pustules — without moderate-to-severe inflammatory or nodulocystic disease Why: Systemic antibiotics for mild acne add resistance and adverse effects without benefit over effective topical first-line therapy. Consider instead: topical retinoid with or without benzoyl peroxide / topical antimicrobials as first-line Source: American Academy of Dermatology (Choosing Wisely, US, 2015).
